The DNA/RNA Sample Kit brings the physical collection materials and illustrated instructions into one at-home package. The original product gallery, instruction spreads, packaging views, and supporting reference images are all retained on this page.
The workflow is designed to keep dry samples separated and protected until they reach QMH. Kit appearance and individual components can change as packaging is updated.
Collection and safety
Collect carefully, let every sample dry completely, and never return used lancets.
Wash and dry your hands before collection. Nail clippings and undyed hair can be placed in the sample bag. Saliva is collected by swabbing both inner cheeks, while blood drops are placed on a sterile swab after cleaning the finger and using a single-use safety lancet.
Saliva and blood swabs must be completely dry before packing. Moist samples can spoil other materials in the bag. Used lancets must be discarded safely and must not be returned with the kit.
Swab the left and right inner cheek for about 30 seconds each.
Allow saliva swabs to dry for at least 45 minutes, or longer in humid conditions.
Allow blood swabs to dry for at least 30 minutes, or longer in humid conditions.
Use preferably undyed hair; body hair can be used when suitable head hair is unavailable.
Digital references
Upload recent photos and a signature in account Settings.
Use a recent, unedited, high-resolution headshot and full-body photo on a neutral background. Only the account holder should appear, with the full body visible and no cropping from group photos.
The signature is used only as an individual reference within the QMH wellness workflow. It is not requested for legal authorization or paperwork. Photos and signatures are uploaded digitally and are not placed in the return package.
If the physical kit is unavailable before a scheduled session, the legacy workflow allows a photo-only process or rescheduling to a later date.
